Supported Living Team Leader

Lead structured independent living supports with a strong focus on quality, routine, and participant outcomes.

We are seeking an experienced leader to help coordinate supported independent living services and guide frontline staff.

The role combines participant support, shift leadership, and communication with families and referrers.

Responsibilities

  • Coordinate day-to-day delivery of supported living supports
  • Mentor and guide support workers on shift practice and documentation
  • Maintain strong communication with participants, families, and stakeholders
  • Monitor routines, risk, and service quality across the roster

Requirements

  • Demonstrated leadership in disability or community support
  • Experience with structured home-based supports and documentation
  • Current worker screening and driver's licence
  • Strong organisational judgement and calm decision-making
