Community Participation Facilitator

Help participants engage in social, recreational, and community-based activities with confidence.

This role focuses on helping participants access the community in ways that are safe, enjoyable, and tied to personal goals.

You will need to be organised, reliable, and comfortable adapting plans to meet participant needs on the day.

Responsibilities

Support participants to attend appointments, programs, and social activities
Encourage confidence, communication, and participation in community life
Provide transport support where required
Maintain clear notes and communicate with the wider team

Requirements

Experience in disability, aged care, youth work, or related human services
Current driver's licence
Confident community-based support practice
A participant-first and strengths-based approach
